Advanced Directives


An Advanced healthcare directive is a legal document that describes a person’s wishes if they should be unable to make their own healthcare decisions. Simply put, an advanced healthcare directive is also known as a “medical directive, advanced healthcare decision, or “living will”. An advanced healthcare directive is generally used when a person develops an illness such as cancer or other minor or serious health related issues.

• Supervision of Care including an appointment of a care manager and companion
• Access to Medical Records
• Employ or Discharge Health Care Providers
• Acute and Long-Term Care including institutionalized and remaining in the residence

• Power to Nominate a Guardian and grant releases
